How to Refer into Inclusion Recovery Hampshire
Referring into our service is simple, secure, and tailored to your needs. You’ll be guided through a secure webpage where you select your local service, your age, and the substance you want support with, ensuring you’re directed to the right referral pathway. If you are referring on behalf of someone else, the webpage will take you through clear steps so we can collect the right information.

Young People:
If you’re under 18, you’ll be taken to a brief form to provide essential information so we can support you appropriately. -
Adults (Self-Referral):
Adults referring themselves will complete a short screening tool to help us understand how substance use is affecting you. If you only need information or educational awareness, you’ll be directed to helpful self-help materials. If the screening shows you would benefit from our support, you’ll be taken to a referral form to provide the details we need to get in touch. -
Family Members or Carers:
If you are a family member or carer, please select that option. You will be asked for brief information about the person you are referring, and we will make contact with them directly to offer support. -
Professionals:
If you’re a professional making a referral, select the professional option and complete the referral information; we’ll then contact the person you’re referring directly.
Our process ensures everyone gets the right support, guidance, or information, quickly and securely.
How will I know that the referral has been submitted?
You will see a confirmation screen when the referral has been submitted.

What to expect
After you have submitted your online referral, a member of our team will aim to contact you within 2 working days.
They will arrange a first appointment with you at a time that suits you, this can be via the telephone or face-to-face.
If you are unsure about anything, just ask. We are happy to answer any questions you may have.
Our services are free – you don’t have to pay for any of the support we offer.
Our services are confidential – you are in control of your information, how it is stored and if it is shared (this will be discussed fully at your first appointment).
